What is PACE?

Programs for All-Inclusive Care of the Elderly (PACE) provide comprehensive, community-based health and social services to frail elderly individuals who qualify for nursing facility placement. PACE allows seniors to receive care in the community rather than in a nursing home or other care facility. The program uses a comprehensive care approach, providing an array of services for a monthly fee that is often lower than the cost of comparable nursing facility care.

Key Features of PACE:

  • Comprehensive Coverage: PACE covers all Medicare and Medicaid services, as well as any additional medically necessary care decided by the PACE team, including prescription drugs.

  • Interdisciplinary Team: Participants receive care from a team of health and social services professionals, including doctors, nurses, therapists, and social workers.

  • PACE Center Services: At the PACE center, participants receive primary care, therapy, meals, recreation, socialization, and personal care in a supportive environment.

  • Home-Based Services: PACE offers skilled care, personal care, and supportive services in the home, including modifications like ramps and grab bars to enhance safety.

  • Community Access: Participants have access to specialists and other providers in the community, ensuring a comprehensive approach to their health care needs.

Services Provided by PACE:

  • Adult day primary care (including doctor and recreational therapy nursing services)

  • Dentistry

  • Emergency services

  • Home care

  • Hospital care

  • Laboratory and x-ray services

  • Meals

  • Nursing home care

  • Nutritional counseling

  • Occupational therapy

  • Physical therapy

  • Preventive care

  • Social work counseling

  • Transportation to the PACE center for activities or medical appointments

Who is PACE For?

  • Older adults who are 55 years or older and live within the PACE service area.

  • Individuals who are frail and qualify for nursing facility placement but wish to remain living safely in the community.

  • Seniors who benefit from a coordinated and comprehensive care plan that includes both medical and social services.
